Your Mission, Should You Choose to Accept:

Provide comprehensive skilled patient care in home health settings, including wound care, catheterization, etc. Be a coach!

Educate patients on exercises and activities they can do at home.

Team up with other healthcare pros to coordinate top-notch patient care.

Conduct thorough patient assessments and maintain accurate electronic health records using Wellsky EMR.
Your Extraordinary Abilities:
-
Minimum of 1 year Home Health or Hospital experience
-
Excellent communication and interpersonal skills to connect with patients, families, and team members.
-
Ability to craft and adjust individualized treatment plans.
-
Strong problem-solving skills to navigate unique patient challenges.
-
Empathy and compassion to support patients throughout their recovery.

Strong organizational skills to juggle multiple patients and tasks.

Have a valid Driver's License and a reliable vehicle

Can lift 50 lbs and can move and position self to perform tasks.

Can pass a 2-step TB test

Hold a current BLS CPR certification

Are willing to undergo and pass background checks
